In brief: Developed with the Report of the NACD Blue Ribbon Commission on Board Evaluation: Improving Director Effectiveness, this sample questionnaire is designed to assist directors with conducting full-board evaluations.  Companies listed on the New York Stock Exchange (NYSE) are required to conduct annual board evaluations. NASDAQ-listed companies and private companies have no such requirements, however the practice of conducting board evaluations has become increasingly common among all company types.
